Minor Participate Waiver

Parent/Guardian Liability Waiver, Assumption of Risk, and Release of Liability for Minor Participants

By registering a minor participant for training with Haines CPR, I certify that I am the minor's parent or legal guardian and have the legal authority to execute this Waiver, Assumption of Risk, and Release of Liability on the minor's behalf.

I acknowledge that participation in First Aid, CPR, AED, Babysitter's Training, Skills Boost courses, professional development activities, and other educational programs offered by Haines CPR may involve physical activity, including practicing CPR compressions, rescue breathing (when applicable), use of an Automated External Defibrillator (AED) trainer, first aid skills, lifting, kneeling, bending, and other hands-on training exercises. I understand that participation carries inherent risks, including but not limited to muscle soreness, fatigue, minor strains, bruising, aggravation of pre-existing medical conditions, or other injuries that may occur despite reasonable precautions.

I certify that, to the best of my knowledge, the minor participant is physically able to participate safely in the training activities. I agree to notify Haines CPR before the training begins of any medical conditions, allergies, injuries, physical limitations, or other circumstances that may affect the minor's ability to participate safely. If the minor's condition changes after submitting this waiver but before the scheduled training, I agree to notify Haines CPR immediately.

I understand that participation is voluntary and that the minor may stop participating in any activity at any time if discomfort or concerns arise. I acknowledge that Haines CPR reserves the right to modify or discontinue a participant's involvement in any activity if it is determined that continued participation may present a safety concern.

On behalf of myself and the minor participant, I voluntarily assume all risks associated with participation in the training, including those arising from ordinary negligence, except where prohibited by applicable law or resulting from gross negligence or willful misconduct.

To the fullest extent permitted by law, I release, waive, and forever discharge Haines CPR, its owner, instructors, employees, contractors, volunteers, agents, successors, and assigns from any claims, demands, actions, damages, liabilities, costs, or expenses arising out of or related to the minor's participation in the training, except to the extent caused by gross negligence or willful misconduct.

I understand that successful completion of a course and the issuance of any certification depend upon the participant satisfactorily completing all applicable course requirements established by Haines CPR and, where applicable, the American Red Cross. Signing this waiver does not guarantee certification.

I acknowledge that photographs or recordings will not be taken or used for promotional purposes without separate permission, unless otherwise authorized by law or agreed to in writing.

By entering my full name, providing my email address, and selecting the required acknowledgment, I certify that:

• I am the parent or legal guardian of the minor participant.
• I have carefully read and understand this Parent/Guardian Liability Waiver, Assumption of Risk, and Release of Liability.
• I have had the opportunity to ask questions before completing registration.
• I voluntarily agree to all terms contained in this waiver on behalf of myself and the minor participant.
• I understand this electronic acknowledgment constitutes my legally binding electronic signature.
